Eligibility Requirements for Program Enrollment

To ensure the safety, success, and readiness of all participants, the following eligibility requirements must be met before enrollment in the program:

1. Minimum Age Requirement: All participants must be at least 16 years of age at the time of enrollment. 

2. Proof of Citizenship: Participants must provide valid documentation demonstrating U.S. citizenship or legal residency.

3. English Competency: Applicants must demonstrate proficiency in English by passing a CNA English Competency test administered by Stand on Promise before admission. 

4. Background check: All participants must provide a current Arizona Fingerprint card 

5. Health Requirements: 

• A licensed board-certified practitioner must complete and sign a physical and mental health clearance form verifying that the individual is physically and mentally capable of participating in the program. This includes documentation of the ability to safely lift at least 50 pounds. 
• A neg TB screening must be provided (with a date not to exceed 90 days before the start date of the program)  
• A neg drug screen is to be done through your primary care provider. 
• Proof of vaccines (Hep B, tdap, MMR, flu) is also required. Failure to meet any of these requirements will result in ineligibility for participation in the program. Additional vaccines may be required by clinical rotation sites. The inability or refusal to meet clinical site requirements will result in expulsion from the program.
